At Franklin County’s Y, a supportive community is a big part of wellness. Our programs help educate the community about healthy habits to prevent chronic diseases from taking hold. The following programs allows us to help bridge the gap between clinical care and community care. The YMCA has the equipment and space to provide high-quality programs along with specified training that allows our staff to become experts in developing healthy habits.

The YMCA isn’t just a gym – it’s a place where people come together to become the best and healthiest version of themselves. We believe that we have an opportunity to improve our collective health by reducing our risk for chronic disease and transforming health and wellness as the primary weapon in our fight against disease.

Blood Pressure Self Monitoring Program

Have you been diagnosed with High Blood Pressure?
Studies shows that the process of recording blood pressure at least twice a month over a period of four months can lower blood pressure in many people with high blood pressure.

Blood Pressure Self-Monitoring is one part of Y-USA’s suite of evidence-based chronic disease prevention programs. Since 2005, the Y has worked to become a leading provider of chronic disease prevention programs to communities across the U.S.

Participants will:
– Take and record their blood pressure at least two times per month
– Attend two consultations a month with a Healthy Heart Ambassador (in person, phone, or Zoom)
– Watch a monthly nutrition education seminar

Free blood pressure cuffs and trainings will be provided!

Lasting Lifestyles Diabetes Prevention

Forming healthy habits can be hard without a support system. We’re here to give you the encouragement you need to eat better, increase your physical activity, lose weight and maintain it! These lifestyle habits can delay or prevent the onset of Diabetes or manage the disease you already have.

A trained lifestyle coach facilitates a small group of participants in learning about healthier eating, physical activity, and other behavior changes. Perfect for participants who have been diagnosed with Metabolic Syndrome, Pre Diabetes, and Type 2 Diabetes, or those who are simply looking for some extra help in achieving healthy habits. We utilize the CDC’s PreventT2 curriculum.

This 16-week program includes one class session and 3 group exercise times a
week. Weekly weigh-ins included. Daytime and Evening classes available.

Prescribe the Y

Prescribe the Y (RxY) is a 12 week Health Care Provider referred wellness program, through the Y, in which participants work towards better health and healing.

Referred patients start with a free 30 minute consultation with one of our Exercise Specialists. During this consultation, your wellness goals and physical needs will be discussed and evaluated to determine which Wellness Track is best for you. Whether you’re on the Weight Loss, Chronic Disease & Pain Management, Strengthening & Conditioning, or Rehabilitation & Recovery Track, YMCA staff will work closely with you to monitor attendance and progress towards your fitness goals.

Program Fee applies for the 12-week special program. YMCA Community Scholarship is available.

RxY INCLUDES:

  • Private consultation to develop an individualized exercise plan to achieve your wellness goals.
  • A YMCA membership and use of facilities 7 days a week for 3 months.
  • Instruction and support from Y Staff and your Exercise Specialist.
  • Ongoing communication, tracked progress, and possible adjustments to your program during your 12 weeks.
  • End of program consult to discuss your next steps towards continuing your wellness.
  • Referring Health Care Providers can receive a participant summary showing progress in your wellness journey.
  • Access to additional evidence-based programs for chronic disease prevention and management.
